PBIS Recognition Resources for 2021-2022 School Year

Each year the Ohio PBIS Showcase is held to recognize the efforts of those districts and buildings implementing PBIS with fidelity and meeting the quality standards created by the Ohio Department of Education. The accomplishments of these schools are celebrated at the annual OLAC-PBIS Recognition Showcase. 

State Support Team Region 5 is accepting electronic submissions or hard-copy documents. The completed application along with data sources to support each feature must be submitted no later than May 1, 2022.

You will want to download the PDF and "Save As" before typing on them.  

*Please note: a team photo in .jpg format is required with each application.  

State Support Team consultants will review applications using the revised January 2022 PBIS Network approved scoring rubrics available in the application. These scoring rubrics are designed to ensure consistent scoring across the state. 


PBIS Binder Tabs

Binder can be submitted electronically or hard copy this year.  However, electronic is strongly suggested.
Using these binder tabs to organize your evidence will assist you as you complete your application. 


Tier I Electronic Binder (Suggested)  It will ask you to make a copy.

Schools and districts that would like to apply for recognition of their implementation efforts for the 2020-2021 school year should complete the 2020-2021 PBIS Application by May 1, 2022. Applicants must complete the Tiered Fidelity Inventory (TFI) Tiered Fidelity Inventory and TFI Walkthrough Tool for the following award levels:

  • Bronze: TFI Tier 1
  • Silver: TFI Tier 1 & 2
  • Gold: TFI Tier 1, 2 & 3

Why do you need a PBIS Action Plan?

An action plan is a list of steps you take to achieve your goal. The two-part question that we want to answer with different types of data is:

  • "Are we doing what we said we were going to do?"
    • Effort Data "How much, and how often?"
    • Fidelity Data "How well?"
  • "Is it benefiting students and families?"
    • Outcome Data
