- This is a great resource is a nice combination of independent living assessment and family engagement. This comes from the Florida Department of Education's Community Based Instruction Guide. The first page is just some background information if a child is going to be in a community-based instruction experience. After that is a survey for parents that asks if a child does specific skills and also with or without assistance. The skills are divided into sections; domestic skills, vocational activities, community access skills, and recreation and leisure. After that parents are then asked to choose community-based activities that are most important as well as community locations the family uses. This form can provide great information for community-based instruction but it also be altered slightly to be very informative to us as it is great data to base an independent living goal.
- This resource was developed by the Arc and is located on the Zarrrow Center's website at the University of Oklahoma. There are 72 items on this assessment which are normed. Students receive a total self-determination score and 4 sub-domains; autonomy, self-regulation, psychological empowerment, and self-realization. Since this test is normed the administrator will need to use the procedural guidelines to convert the scores. Below are a few more resources to help score and administer the assessment. The results can help teams see the strengths and areas of need for a student's self-determination. From there, teams can develop and monitor appropriate transition goals for students.
