Indiana University Secondary Transition Resource Center Transition Planning Best Practices
  • Indiana University has developed several great transition resources. One you might be most familiar with is the Indiana University Transition Assessment Matrix. This handout is another great resource. It outlines what they refer to as the five best practices for transition planning. This handout lists the best practice and gives a short description of that practice. This is a great resource to allow for self-reflection on our current practices, which is a strength of yours, and which is one you can improve on. 


Resource Mapping Worksheet
  • We are all familiar with NTATC and all their work. This is a great resource that can be used for many purposes. One of these can be to see what current resources you have in place and which ones you might want to explore. Another nice feature is that there is a column to show which grade level these resources are available in. This is another nice way to audit what grade levels may have more or less resources to help you map out how you want your program to expand. Note these resources are aligned to each of the areas of Pre-Employment Transition Services to help organize the services. 


Resource Mapping Flow of Services 
  • Here is another resource to help identify services and goals for students at different age ranges. This graphic organizer has a place for the goal of each year as well as how this goal will be met. Below those two rows, there are spaces for the multiple agencies that may involved with a student to identify each of their responsibilities in helping the student meet their goal which is listed above. This is a great resource to help teams very specific with the what, how, and who is responsible for each year of a student’s transition plan. TAGG Middle School Assessment Field Test 
  • The TAGG assessment is a great assessment for the Zarrow Center at the University of Oklahoma. It was originally for 9-12 grade students. They have developed a middle school test and are looking for teachers to field test it. There is a small monetary incentive as well. Check out the flyer for more information.
