Transition Tidbits October 18, 2021

Hi Everyone,

One last week of Student-Centered Planning resources aligned to the Taxonomy of Transition Programming. With that said we have four great resources to make our plans more student-centered. Below are some great events that are coming up in October and November. Finally, at the bottom, be sure to check out some informational brochures from our friends at OOD.

Both of the resources below have been shared in the past but they both align so well and are critical for Student-Centered Planning. Even if you are not a part of a Multi-Agency or Transition Assessment Planning team, these documents can be used by anyone to facilitate a student-centered transition plan. As you browse these documents; think about all the benefits of planning transition assessments but how much time do we actually spend doing it?

Note- This document also aligns well with the backward planning template, shared on 9/27

5 Key Principles of Person-Centered Planning

  • Nice, short video from the Ohio Department of Developmental Disabilities. The video is about the DD service plan but many of the concepts can also apply to person-centered IEP planning. The video is directed at individuals with a disability and provides them with real-life examples of each of the 5 key parts. It helps individuals with a disability understand what specific groups, such as friends or family, can do for them. The video also does a nice job of explaining when the individual with a disability should speak up in their meetings and what they should say.

Student-Led IEP PowerPoint Templates

  • If you review the Student-Centered Planning section of the Taxonomy of Transition Planning, you will notice that one of the activities/ideas talks about student-led IEP’s. This website is a link to some Powerpoint slides that can be used for a student to fill out in order to guide their presentation during their student-led IEP.
