Preparing for the ISP/IEP

• Here is a quick take 5 videos from OCALI. Remember these are short videos with multiple links attached to them. This one shows how schools can use an individual service plan to inform their IEP. The video also discusses how both agencies can use Charting The Lifecourse to inform their planning. Remember to take a look at the 5 links below the video as well. 

Healthcare Transitions Readiness 

• One more take 5 from OCALI. This one is a great engagement video for parents. The video will help teams identify goals, strategies, and resources that can be added to a youth’s IEP to help the child develop the skills they need to be in charge of their own health care. Again there are 5 great resources below that help to build a family's knowledge and help the youth develop more independence in their health care. 

Independent Living Checklist 

• Another nice resource that aligns with the other two shared this week. Sometimes it is difficult to identify when a student needs an independent living goal or even what that goal should be. This is a really great checklist that can be used by parents and teachers to help plan goals for students based on their strengths and weaknesses. The checklist is separated into sections as well which helps teams identify larger areas of need for students.
