Transition Tidbits April 11, 2022


Hi Everyone,


This week we are going to stay in the Interagency Collaboration section of the Taxonomy of Transition Programming. Our first two resources align with that section and our third resource is a general resource but a really good one from the Zarrow Institute. We are going to take next Monday off since it is Easter Monday. We hope you all have a great Easter and get to enjoy some time off. 



Interagency Agreement Toolkit 

  • It is hard to have agencies working together without a formal agreement on how these services will be provided to students. This document is full of sample language for a formal agreement between a school and outside agencies. There are also questions for each agency to consider about how services will be delivered. This document is a nice template for anyone wishing to establish their own agreement. 


Local Discussion Prompts for Implementation of an Interagency Agreement 

  • Another real good template for agencies that are both providing transition services to students. This document has categories of questions and many examples of follow-up questions to get more information. The questions are also organized into a chart which has a column to divide responsibility and take notes specific to the education or vocational rehabilitation agencies. 


Student-directed Transition Planning 

  • Another great resource from the Zarrow Institute. Our goal is to have transition planning be student-directed. However, students may not always have the skills needed to make this type of planning possible. This is a Free course of 8 lessons on student-directed transition planning. Educators can use these courses to teach their students the skills needed to participate in Transition Planning. Lessons contain teaching materials, a presentation, a teacher’s guide, and lesson activities.
